On January 1, 2024, two trusted organizations—AxessPointe Community Health Centers, Inc. and Family & Community Services, Inc.—came together to form Axess Family Services.

This new identity reflects our expanded mission: to open pathways to wellbeing by connecting communities to health and supportive services.
