A partnership bridging community health and social service

On January 1, 2024, AxessPointe Community Health Centers, Inc. and Family & Community Services, Inc. merged creating Axess Family Services, Inc.

AxessPointe is a Federally Qualified Health Center that provides comprehensive primary care to 20,000 patients annually regardless of their insurance status or ability to pay. Family & Community Services is a social service organization that provides a wide range of community support programs that address homelessness and housing issues, transportation gaps, food insecurity, domestic violence, behavioral health and addiction, and gaps in care for youth seniors and veterans to over 200,000 individuals each year. This dynamic partnership enhances access to comprehensive healthcare and behavioral healthcare, and social services across Summit, Portage, Trumbull, and Mahoning counties, and a wide-range of social services throughout Northeast Ohio, and parts of Pennsylvania and Michigan. Together, Axess Family Services will create a person-centered approach to health and well-being, ensuring that individuals receive the care they need, both medically and socially, advancing health equity for all.
