On January 1, 2024, AxessPointe Community Health Centers, Inc. and Family & Community Services, Inc., merged to form Axess Family Services.

This partnership combines high-quality primary care with a broad range of social services including housing support, food access, behavioral health, and more, serving communities across Northeast Ohio and portions of Pennsylvania and Michigan. Together, Axess Family Services is advancing a person-centered approach to care, addressing both medical and social needs to promote health equity for all.
