Insurance Enrollment Assistance

Need help navigating Medicaid or Medicare? We provide free assistance in enrolling in both of these plans. Our Certified Application Counselors (CAC) meet with patients and community members for a private, one-on-one conversation about how to find a healthcare plan that fits their needs and budget.

We are happy to offer this service for free, though it helps to make an appointment in advance. The CAC you meet with will tell you what to bring to the appointment, such as your ID, proof of income, and address verification.

AxessPointe also offers a sliding fee scale based on household income and family size.

Connections to Other Resources

Our Community Health Workers (CHWs) can connect our patients with other resources in the community, such as transportation assistance, housing support, and utility assistance. And because AxessPointe is a part of Axess Family Services, patients can now receive help from a number of social services programs under our umbrella that address homelessness and housing issues, transportation gaps, food insecurity, domestic violence, behavioral health and addiction, and gaps in care for youth, seniors, and veterans. Medication-Assisted Treatment (MAT) Services

Medication-assisted treatment (MAT) uses prescription medications, in conjunction with counseling, to treat substance abuse disorders. AxessPointe utilizes Vivitrol, which prevents a relapse to opioids once a person has completed detox. 

Patients receive medication and counseling services within the same building. The medication is prescribed by AxessPointe and the counseling services are provided by Family & Community Services, Inc. Both the medication and the counseling are required pieces for MAT to be successful.

MAT is a big part of recovery but staying sober depends on more than just medications. Showing our clients we have faith in them allows them to see in the midst of difficulty, there is an opportunity for them to live their best lives.
