Axess Family Services (AFS) provides access to comprehensive healthcare, behavioral health services, and social services. AFS serves over 350,000 adults and children annually and offers a wide-range of social services throughout Northeast Ohio, and parts of Pennsylvania and Michigan.
AFS is committed to a person-centered approach to health and well-being, ensuring that individuals receive the care they need, both medically and socially, advancing health equity for all.
Our History
Axess Family Services (AFS) was formed on January 1, 2024, through the merger of two trusted organizations— Family & Community Services, Inc. (FCS) and AxessPointe Community Health Centers (API). This partnership unites decades of experience in social services and community-based healthcare under one shared vision: to provide comprehensive, person-centered care that strengthens individuals, families, and communities.
Family & Community Services began in 1941 as the Catholic Charities Bureau of the Ravenna Deanery, with just two programs and two staff members. Over the decades, FCS evolved into one of the region’s most extensive social service organizations, operating more than 70 programs across Ohio, Pennsylvania, and Michigan. Its wide-ranging services address homelessness and housing, food insecurity, transportation, domestic violence, behavioral health, addiction, and support for youth, seniors, and veterans.
AxessPointe Community Health Centers, established in 1995 as Northeast Ohio’s first Federally Qualified Health Center (FQHC), brought accessible, affordable healthcare to underserved populations. With locations throughout Summit and Portage Counties, AxessPointe provides comprehensive primary medical, dental, behavioral health, and pharmacy services for patients of all ages, regardless of their ability to pay. Its focus on integrated, team-based care and addressing social determinants of health has made AxessPointe a cornerstone of community health and wellness.
Together as Axess Family Services, the organization combines the strengths of Family & Community Services and AxessPointe Community Health Centers to create a fully integrated network of care that unites healthcare and social services under one name. Axess Family Services is committed to forging new paths, creating innovative solutions, and improving health equity for all. Each year, the organization serves more than 350,000 adults and children, supported by over 3,800 dedicated volunteers who help bring its guiding principle to life: Go Forth and Do Good.
