Starting services at Children’s Advantage begins with a few important steps to help you feel prepared and informed. Our New Client Information Packet provides everything you need to understand your rights, responsibilities, and what to expect as a client. Inside, you’ll find details about orientation, confidentiality and HIPAA compliance, financial policies, guardian/parental rights, and more. This packet is designed to ensure you have clear guidance and peace of mind as you begin your journey with us.
New Client Information Form
The New Client Information Form must be filled out and returned to Children’s Advantage either by dropping it off or mailing it to our main office at 771 N. Freedom St., Ravenna, Ohio 44266, or the Family Center at 5968 New Milford Road, Ravenna, Ohio 44266. Submitting this form ensures that we have the necessary information to provide you and your family with the best possible care.

We accept Medicaid, Self Pay and allow paying on a sliding scale. We work with you to find the best method for you to pay for treatment. We are in network with most insurances. Call us to verify insurance.
