Axess Family Services (AFS) is proud to be a Health Resources and Services Administration (HRSA)-funded organization. As part of this designation, we comply with all applicable federal regulations and requirements to ensure accessible, affordable, and high-quality care for the individuals and communities we serve for all of our healthcare programming.

Federally Qualified Health Center (FQHC) Status

AFS operates a Federally Qualified Health Center (FQHC) under HRSA guidelines as well as other non-HRSA funded programs. This designation helps us provide comprehensive primary care services to underserved populations, regardless of their ability to pay.

As an FQHC, we are committed to:

  • Providing services on a sliding fee scale based on income and family size
  • Serving all individuals, including those uninsured or underinsured
  • Meeting rigorous quality and accountability standards
  • Offering culturally and linguistically appropriate services

Notice to Patients

AFS receives funding from the U.S. Department of Health and Human Services and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals. This means AFS’ healthcare programs and its providers are covered under the Federal Tort Claims Act (FTCA).
