Axess Family Services’ Behavioral Health Programs offer the most advanced behavioral health treatments for adolescents, adults, and seniors. AFS ensures that individuals and families are put on a path to healing. As a comprehensive service provider, the agency also provides recovery housing and support for both men and women in Portage County.
Our programs are staffed by psychiatrists, nurse practitioners, nurses, licensed therapists, and case managers. Our team is dedicated to providing the immediate help an individual might need. Our Services
Our behavioral health programs provide diagnosis, treatments, and education to adolescents, adults, and seniors. We also offer court-ordered programs, including Driver Education Class, Think About Your Future Diversion Program, and BIC-60 Program.
Our providers have many specialties, including trauma-informed care, eye movement desensitization and reprocessing (EMDR), dialectical behavioral therapy (DBT), cognitive behavioral therapy (CBT), and grief counseling, among many other specialties.
Who We Serve
Outpatient Counseling with our Behavioral Health Programs serve all ages, including children, teens, and seniors. We accept Medicaid and most major insurances, as well as provider, court, and self-referrals. We provide a wide range of behavioral health services, including individual, family, and couples counseling, group counseling, psychiatry, diagnostic assessments, senior outreach, court-ordered diversion programs, substance use, and intensive outpatient programs.

Diagnostic Assessment

Individual Counseling

Group Counseling
Healing can happen in connection with others. Our group counseling sessions offer a supportive environment where individuals share, learn, and grow together. Led by experienced therapists, groups focus on topics like anxiety, depression, relationships, and recovery so you can find strength and understanding in a safe, guided space. Case Management Services

Peer Support Services
Sometimes the best support comes from a peer who truly understands. Our certified peer support specialists have lived experience with behavioral health challenges and are trained to offer hope, encouragement, and practical guidance.
Telemedicine & Telehealth
Your care, on your schedule, from wherever you are. Our secure telehealth services connect you to therapists, nurses, and other providers through video or phone, so you can receive high-quality support in the comfort of your own space. Easy, convenient, and confidential.

Drug Screen & Lab Services
Recovery, Empowerment, and Action Creating Hope (REACH) Program
The REACH program empowers clients to unite their efforts in providing safe and stable environments for children and adults through an individualized approach. AxessPointe Community Health Center provides counseling services for mental health, trauma, substance abuse, domestic violence, and parenting issues to families referred to the program due to involvement with Child Protective Services (CPS).
Benefit (BRIDGE) Program
The BRIDGE program offers mental health and substance abuse counseling, financial literacy counseling, and case management services to Portage County adults transitioning from public assistance to self-sufficiency. To be eligible, clients must submit an application to Job & Family Services. Those who meet the program criteria are referred by JFS to AxessPointe Community Health Center for services.
Senior Track – Adult Protective Services (APS)
The Senior Track program provides in-home mental health counseling, substance abuse counseling, and case management services for Portage County residents age 60 and older. Eligible clients are referred by Portage County Job & Family Services Adult Protective staff, and their needs are assessed by AxessPointe Community Health Center outpatient clinical staff to determine appropriate services and treatment programs.
Comprehensive Case Management and Employment Program (CCMEP)
CCMEP provides integrated case management and employment services to Portage County residents ages 14–24, helping participants overcome barriers to employment and develop skills sought by local employers. AxessPointe Community Health Center offers counseling, case management, financial literacy, and assistance with re-entry programs, transitional housing, and emergency housing. Clients are referred through Job & Family Services and complete a comprehensive diagnostic assessment to identify individual needs.
