Take an Active Role in Your Health
Regular medical check-ups help both you and your doctor stay up to date with your health. If your family has a history of certain medical conditions, regular visits can inform you of your risk for these conditions and identify potential problems before they start. Common screenings may include checks for blood pressure, cholesterol, and certain cancers.
Rely on our physicians and nurse practitioners to keep track of your long-term health.

Before Your Appointment
Before your appointment, it is important to write down any questions you might have for your doctor. There is no question that is too small. Even discussing your eating, sleeping, and exercise habits, alongside any changes to them, can help your doctor better understand the state of your health. Be sure to tell your doctor if you know of any illnesses in your family’s history and ask about staying up to date with your vaccinations and health screenings.
Not sure what questions you should ask during your visit? The Agency for Healthcare Research and Quality has information for patients and offers tips on questions to ask during your check-up:
No Insurance? No Problem.
We want you to receive the care you need, even if you do not have insurance or enough coverage. We accept Medicaid, Medicare, and most private insurance plans. If you don’t have insurance, we have a sliding-fee scale that determines cost based on income and family size. Learn more about our sliding-fee scale or fill out our Sliding-Fee Discount Program (SFDP) form below.
